Abstract:
Immediate tooth replacement therapy has become a mainstream treatment modality for single and multiple tooth implants in the aesthetic zone. The balance between achieving primary stability, implant diameter, and gap distance is a delicate dance yet essential for survival and aesthetics. However, aesthetic risks and complications exist such as apical socket perforation, loss of labial plate thickness over time, and loss of the interdental papilla due to proximity issues with tooth-to-implant distance following placement.
An inverted body-shift implant design (INVERTAâ) will be presented and supported by pre-clinical [animal histology] and clinical [human] studies that scientifically supports, validates, and reinforces the biologic principles in design that ultimately enhances aesthetic outcomes in modern day implantology.
Learning Objectives:
[1] Understand the implant complications and risks in the aesthetic zone
[2] Understand the biology behind circumferential bone volume for long term maintenance to prevent ridge collapse, recession, and papillae loss
[3] Understand the inverted body-shift concept in macro implant design
The use of coronal “body-shift” design implants allows a greater gap distance for blood clot formation and eventually new labial bone maturation . This predictably & reproducibly results in better soft tissues support , papilla preservation and ultimate aesthetics. Simultaneously the wider apical end ensures high insertion torque values at the time of implant placement for immediate function. The 12-degree angle which is incorporated into the body of these implants allows for sub-crestal angular correction and for a screw retained restoration. With good primary stability, these reverse tapered implants can also be loaded immediately with SameDay® Teeth for immediate reconstruction & immediate function. In the single implant case, it is possible to deliver the permanent all ceramic zirconia screw retained restoration routinely & predictably within 4 hours. In cases of multiple implants, it is possible to deliver acrylic screw retained temporary teeth within 4 hours and permanent all-ceramic or metal-ceramic teeth within 7 days. High treatment acceptance and patient satisfaction are the most important advantages of this protocol. By avoiding excessive bone grafting with the use of Site Specific implants placed in available native bone, the gain in time for the patient implies an economic & time benefit which is important especially for professionally and/or socially active patients.
