The development of a digital workflow based on new medical 3D-devices has changed the world of dental treatment. Based on the history of digital dentistry and the development of the digital workflow it will be shown, that these new tools have led directly to surgical innovations like development of new surgical devices as laser osteotomy or producing 3D-printed dental implants to achieve big improvements of the esthetic and functional results in oral rehabilitations. But optimizing the treatment result faces an increasing complexicity of the treatment workflow; increasing costs are an additional side effect. Innovations have created not only new possibilities, but also new problems and hazards which leads to the demand not only to develop other new technologies, but also to include more and more fundamental biological principals in reconstructive augmentative oral surgery. Out of this, innovative augmentative surgival techniques have been developed like using dentinum for block augmentation or using the forces being generated by orthodontic extrusion to stimulate bone growth. Out of the presentation it should be get clear, that the goal should be to imitate nature more and more and not to replace biological approaches by technical ones. As a consequence of such a paradigm old techniques like osseofixation could get a revival and could be an alternative to osseointegration. But if it get possible to use new digital technologies on the base and combined with physiologic principles, then also new application fields could be established for the benefit of the patients. New possibilities of 3D-operation planning and CADCAM-based manufacturing of patient-specific implants in dental implantology will be presented. And by this, even in complex patients who could not be treated up to now implantological restorations will be made possible combining 3D-technologies with biological support by autologous blood concentrates and nutritional supplements.
